| The Santa Clara Fire Department Hazardous
Materials Division accepts the Santa Clara County Uniform
Documents for most submittals. These documents are maintained
on the Internet.
The forms on this website are in Portable Document Format

The Santa Clara Fire Department also maintains other documents
that are available for downloading or on-line submission.

below that are highlighted are available online. Please click

- Hazardous Materials
Business Plan Forms - Hazardous Materials Management
Plans/Business Plans ("Business Plans") are required of
all companies storing or using hazardous materials above
threshold quantities pursuant California Health and Safety
Code, Section 25503.5. The threshold quantities are as follows:
- 500 pounds or more for solids
- 55 gallons or more for liquids
- 200 cubic feet or more for compressed gases
The Business Plan includes an inventory statement, a
site map showing the location of hazardous materials,
an emergency response and contingency plan, an employee
training plan, and general information about the facility.
The Business Plan must be kept on site in a readily accessible
area. The company must also review the business plan at
least once every two years. Copies of the inventory statement,
site map, and facility information must be submitted to
the Santa Clara Fire Department annually. These are due
within two weeks of the annual Fire Department inspection.
The available forms may be filled out on-line, printed
out and mailed to 1675 Lincoln Steet, Santa Clara, CA
95050. You may also e-mail the completed forms. Please
note that the requirement for original signatures on some
of the forms (Business Owner/Operator Identification and,
for companies subject to the Emergency Planning and Community
Right to Know Act, the Hazardous Materials Inventory forms)
will necessitate that you print out copies of these forms,
sign them and mail them to the above address.

Request for Incident and/or Inspection Reports (PDF)
- this form is used to request copies of incident reports
or inspection reports. While this form is not interactive
it may, however, be filled out online, then printed and
mailed, faxed or delivered to the Hazardous Materials Division
office.

Hazardous Waste Generator Permit Application (PDF) -
this form is required of businesses that generate hazardous
waste, but do not store or use hazardous materials or waste
above the Hazardous Materials Business Plan reporting threshold
(see above). This form is available at our office at 1675
Lincoln Street in Santa Clara. It must be signed and mailed
or delivered to the Hazardous Materials Division.
- Onsite Hazardous Waste Treatment Notification Form (DTSC
1772) - this form is required of hazardous waste generators
that treat hazardous waste under Conditional Exemption,
Conditional Authorization or Permit By Rule. It is available
at our office at 1675 Lincoln Street in Santa Clara, or
it is available online from the California
Environmental Protection Agency (Hwf1772f and Hwf1772u).
It must be signed and mailed or delivered to the Hazardous
Materials Division when complete.
- Inspection Report for Generators - this document is the
inspection checklist that Division personnel use when conducting
inspections of hazardous waste generators and tiered permitting
facilities. Businesses may find it useful for pre-inspection
audits. This form is available at our office at 1675 Lincoln
Street in Santa Clara.
- Closure
Plan for Hazardous Materials Facilities (PDF) - this
form is for facilities that used hazardous materials and
are either closing or eliminating the use of hazardous materials
in certain areas. A closure plan is required to assure that
the hazardous materials were removed or disposed of in a
way that minimizes the threat to public health and safety.
This form is available online, or it may be picked up at

Clara Fire Department also accepts the Santa Clara County
Uniform Document version of this form. The forms on this

- Underground Tank Installation
Guidelines - These guidelines are for facilities that
wish to install underground tanks containing hazardous materials.
These guidelines are also available at our office at 1675

- Underground Storage Tank Removal Policy - this document
describes the permitting process and removal procedures
for removing underground storage tanks that contained hazardous
materials. This document is available at our office at 1675
Lincoln Street in Santa Clara.
- Chemical
Labeling Guidelines (PDF) - The guidelines are to assist
businesses in labeling chemical baths, tanks, containers,
and storage areas in a manner that satisfies the requirements
of the Santa Clara Municipal Fire Code, Section 8001.7.
